Required Client Forms

Scheduling & Payment

    • Payments will be taken prior to session via Square (credit card or on file), cash or check.
    • A 24-hour notice is requested for cancellation of an appointment. A 50% cancellation fee is due before your next appointment. The fee will be waived in case of oncoming illness or exposure.
    • Due to increased cleaning measures, appointment times are as scheduled and cannot extend beyond the stated time to accommodate late arrivals.
    • At my discretion I will provide receipts for payments of treatments by date to insurance companies or attorneys seeking reimbursement.

Right of Refusal

I reserve the right to refuse service to any person for any reason including no reason at all, at any time up to and including during the treatment.

 

COVID-19 Safety Policies & Procedures

Effective date: July 13, 2020  

 

The following policies and procedures are being instituted to help increase safety measures to deter the transmission of COVID-19. Bodywork should be an effective and pleasurable experience and is needed now more than ever. By complying with these measures, our safety and your treatment will have the best possible means of continuing.

Please review the following policies. Failure to follow these measures will result in loss of access to the studio.

 

Studio Cleaning

    • Massage room equipment and surfaces are disinfected before and after each appointment.
    • The bathroom will be disinfected after each client.
    • Linens will be washed according to CT COVID requirements.
    • For your increased safety the studio is equipped with a medical-grade HVAC system sanitizer.

Required Client Forms & Procedures

    • Our COVID-19 release form is to be filled out online prior to each treatment.
    • Masks must be up prior to entering the studio. You must enter through the back door and proceed directly to your treatment room. The Front room and waiting areas are closed until further notice.
    • Masks must be kept on throughout treatment and at all times in the studio.
    • Clients subject to a temperature check prior to treatment, temps over 100.4ºF will be declined treatment.
    • Please observe social distancing when possible.
